Academic Urology & Urogynecology of Arizona
Academic Urology & Urogynecology of Arizona is a medical practice with seven locations in Arizona in the cities of Anthem, Buckeye, Phoenix, Sun City, Sun City West, Litchfield Park, and Wickenburg.
On or about May 22, 2025, Academic Urology detected unauthorized access to their network.
After an investigation and manual document review, it was determined on that between May 18, 2025 and May 22, 2025, certain files containing personal and or protected health information may have been subject to unauthorized access or acquisition.
According to a filing with the Office of the Maine Attorney General, over 73,000 people have been affected. Notices dated February 12, 2026 are being sent.

Terra Holdings
Terra Holdings deals with luxury residential sales, commercial real estate services, appraisals, and insurance in New York and Florida.
According to a filing with the office of the Vermont Attorney General, “On February 13, 2025, Terra Holdings learned of suspicious activity on certain systems in its environment.”
An investigation was conducted and found that certain files were accessed or taken without authorization between February 11, 2025, to February 13, 2025.
It is unknown how many have been affected by this breach.
Northstar Fertility Partners
Northstar Fertility Partners LLC is a Boston-based network of fertility clinics, hospital groups, and doctors.
According to a filing with the Office of the New Hampshire Attorney General, “Northstar learned that a security incident occurred at a third-party vendor, Salesloft, that allowed unauthorized access to a database hosted by Salesforce. The incident, which reportedly affected hundreds of organizations, occurred between August 12, 2025 and August 17, 2025.”
An investigation was conducted and determined that the affected database may have contained certain personal information, including names and driver’s license numbers.
Northstar began mailing notification letters in February 2026.
