McShane & Brady, LLC is investigating a data breach that occurred on April 26, 2026 from Meridian Health Plan of Illinois.

An unauthorized individual gained access to certain member information through the Meridian provider portal.

According to the notice on their website, “The information involved varies by individual, but may include member names, Member ID number, dates of birth, contact information, and limited eligibility or claims-related details.”

This breach has affected approximately 21,000 individuals.
