McShane & Brady, LLC is investigating a data breach from VITAS Hospice Services, LLC (“VITAS”), the largest for-profit hospice chain in the United States.

VITAS discovered access was gained to one of its vendors accounts by an unauthorized individual on October 25.   An investigation was conducted and discovered that a breach occurred between September 21, 2025, to October 27, 2025.

Names, addresses, phone numbers, dates of birth, Social Security numbers, driver’s license numbers, diagnoses, medications, lab results, conditions, treatment information, health insurance information, and other personal information may have been compromised.

According to a filing with the U.S. Department of Health and Human Services, this breach has affected over 319,000 individuals.
