Outcomes One

Outcomes One Inc, a healthcare services provider that partners with health plans across the United States to deliver medication therapy management and adherence programs, filed a notice of a data breach with the Office of the Attorney General of California.

Outcomes discovered that on July 1, 2025, an employee’s email account was accessed by an unauthorized third party following a phishing incident.

On July 17, 2025, an investigation determined the accessed items included individuals’ information.  Accessed items included name and one or more of the following:
• demographic information, including date of birth, gender, phone number and/or state.
• health insurance information, including health plan name, service date, health care ID, diagnosis, and/or other medication information.

Notification letters are being sent to those affected by this breach.

Modernizing Medicine

Modernizing Medicine, a provider of specialty-specific electronic health record software, has announced a July 2025 security incident involving theft of data from its systems.

An investigation was conducted and determined and concluded that on July 29, 2025, unauthorized access to its servers was confirmed between July 9, 2025, and July 10, 2025.

Information disclosed included contained personal and protected health information such as full names, dates of birth, addresses, phone numbers, email addresses, Social Security numbers, bank/financial account information, record numbers, patient account numbers, provider and practice names, billing and diagnostic codes, and health insurance information.

Notification letters started to be mailed to the affected individuals on October 17, 2025.
