Fyzical Therapy & Balance Centers

Fyzical Acquisition Holdings LLC, the parent company of Fyzical Therapy & Balance Centers, suffered a data breach involving unauthorized access to the personal and protected health information of its patients.

Fyzical Therapy & Balance Centers is a physical therapy franchise with more than 500 locations in 46 U.S. states, including local facilites in Independence, MO and Shawnee, KS.

On or around December 9, 2024, activity was identified within its email system.  An investigation was conducted and found that personal information disclosed included names, dates of birth, Social Security numbers, driver’s license numbers, state IDs, financial account information, credit card information, medical information, and health insurance information.

It is unknown how many have been affected by this breach.

SAX, LLP

SAX LLP (“SAX”),an accounting, tax, and advisory firm based in New Jersey recently announced a data breach.

The SAX data breach involved sensitive personal information belonging to over 225,000 individuals.

According to a filing with the Office of the Maine Attorney General, “On August 7, 2024, Sax identified unusual activity involving its network.”   The filing goes on to say, “forensic investigation determined that certain Sax data may have been accessed or acquired without authorization in connection with this incident.”

On December 16, 2025, SAX began mailing data breach notification letters to impacted individuals.

Aflac

Aflac experienced a cyberattack in June 2025 that was reported to the HHS’ Office for Civil Rights on August 8, 2025.

Aflac has now confirmed that the data breach has affected approximately 26,500,000 individuals. It has now been confirmed that the compromised data includes members’ names, addresses, dates of birth, government-issued ID numbers such as passport and state ID card numbers, driver’s license numbers, Social Security numbers, medical information, and health insurance information.

Aflac specializes in supplemental health insurance coverage for medical expenses not covered by a primary insurance provider.
